Food Log Form

One step to increasing your awareness of what you eat is to log the foods you eat for a few days. Regularly recording your intake is also one of the approaches your Nutrition Coach may recommend to you to enhance accountability. You may download the “Food Log”, fill it out, and take it with you to any nutrition appointment.
